   That's a good question.  And you may spend quite some time finding it 
out.  I've seen it in one form or another for years.  I onec typeset it 
for some friends.  I also cleaned up the ASCII version for inclusion in 
one of the SAAAs.

   But finding out who originated it may be difficult.

I typed in the original (r-rated) ASCII version.  But of course I didn't
come up with the idea in the first place.
