OK.. This is how I interpret it, although the use of the words is certainly
not limited by the way I define them here; you can say each word anywhere.

# after a number, or on a telephone:  It's called "Pound," because when # is
after a number, it's an abbreviation for "pound" or "lb." -- lb. being an
abbreviation for LAZY BUM. ;) -- so I weigh more than 100# but less than
200#.

# before a number: It's a "Number Sign."  It's used as an abbrev. for the
word "Number," or "No." or "Num."

# by itself: a "Hash."  I think this is because of its shape.

If you forget any of this, you can call it "that little tic-tac-toe board
thingy" and people will probably understand what you mean. ;)
