Looks like tab damage to me.

Actually, it looked fine to me until I read your post. Apparently my
news reader interprets those tabs the same way as the software that the
artist used.

Here is the picture again, with tabs replaced with numbers. One number
for each tab ("111" means one tab that took the same room as three
spaces, "111122222222" means that there were two tabs right after each
other, where the first one took four spaces and the second took eight):
